Support Payment Notice for
Whitefish Lake First Nation #459 Members

The Whitefish Lake First Nation #459 (“WLFN”) is providing support to WLFN #459 members
that have been negatively impacted by the 2023 Wildfires.
Schedule “4” of the Agricultural Benefits Settlement Trust Agreement permits expenditures for
Health and Public Safety and allows the Nation to make payments to individual members for
assistance for which all Members are eligible to apply.
Upon application, WLFN will provide a support payment to WLFN members (the “Support
Payment”). The Support Payment will not exceed $1,000 per member.
Eligibility Requirements for the Support Payment
To be eligible for the Support Payment you must meet the following requirements:

Be a member of WLFN #459; and
-
Have been negatively impacted by the 2023 Wildfires
How do I apply for the Support Payment?
To apply for the Support Payment, each adult member must complete and submit one of the application forms. There are two types of forms:

Parent/Guardian Form: The primary Parent/Guardian will use this form, and will include all band member minors in their care. The other spouse or primary caregiver will fill out the individuals form.

Individual Form: This form is for singles with no children, and/or for the other spouse.
***If a family member is under the age of 18, The minor’s guardian must complete an
Acknowledgment of Trustee, Release, and Indemnity Agreement.
Application forms will be available for download on the official WFLFN website
(www.whitefish459.com). An Acknowledgment of Trustee, Release and Indemnity Agreement
for minors will also be available for download.
Completed application forms can be submitted to: 2023applications@whitefishadmin.ca
For on-reserve members, application forms will also be made available at the Band Office and
Atikameg Confectionery.

will not be processed until after the holidays.
Parent/Guardian Application Form
Please use this application if you have children, or have children in your care. Please only include minors who are band members. For families with two main caregivers, only one caregiver can apply for the minors/children.
The other caregiver must use the individual form.

Even if the band has your banking information, you still need to provide a void cheque with your application if you're wanting your support payment direct deposit.
Individual Form
The Individual Application form is for single adults (18+) who have no children in their care. This form is also for the other main caregiver of the family.
